1. Select 'Program content' at the top of your page
2. Select 'Outcome Measure'
3. Select 'New Outcome Measure'
4. Add the outcome measure details - name, category (can be defined on the settings page) and a description
5. Click 'Create Questionnaire'
6. Add the details of your outcome measure
